Introduction

Helping the ElderlyHACC helps eligible City residents remain at home instead of the possibility of going into residential care.

Frail aged (on a pension) or a younger person on a disability support pension, living in postcodes 5000 or 5006 may apply for these services. The resident may refer themselves, or be referred by family members, a GP, a friend or neighbour.

What services do we provide?

● Domestic assistance (such as cleaning or unaccompanied shopping).
● Social support (such as accompanied shopping).Volunteer - Home Books
● Home maintenance (basic jobs such as replacing tap washers, light globes or light gardening).
● Home modification (supply and install hand rails in toilet or bathroom).
● Transport (to medical appointments within the City, or social activities).
● Person care (such as assistance with showering, dressing).

All of our services, apart from assessments, attract a fee per session, depending on your income level. The cost of materials for home maintenance or home modification will be passed on to the consumer.

Assessment visit

In order to determine the services the resident requires, we conduct a home visit shortly after being contacted.

If we can help, we will then look to match the needs of the resident with the services we can provide. A service/care plan will be developed for the resident.

Advocate

The Resident may wish to have an advocate present at an assessment visit. This advocate may be a member of your family, a friend. Their role would be to provide support to you and ensure we understand your needs. Please see our 'Your Rights' brochure for further information.

Re-assessment

If the resident's state of health and well-being changes at any time and is receiving services from us, you may request a review which might vary the type or frequency of services we provide.
