Food Business Notification Form

Under Standard 3.2.2 Food Safety Practices and General Requirements, a food business must, before the food business commences any food handling operations, notify Council of the following information through the completion of a Food Business Notification Form:

* Contact details for your business that include the business trading name and the name and business address of the owner of the business.
* Information on the nature of the business, which includes information on the type of business and the foods that it makes or supplies.
* The location of your premise.

It is important that food businesses complete a Food Business Notification Form so Council is aware of food businesses in the City and how to contact them as well as to provide guidance and advice on food safety risks associated with these businesses.

A Food Business Notification Form only needs to be completed and lodged with Council once as long as the information provided is still correct. However, when there are changes to, for example, the name, address or owner of the business, or to what it does or the food it deals with, then you must complete another Food Business Notification Form before these changes take place.

Section 86 (1) of the Food Act 2001 states: The proprietor of a food business must not conduct the food business unless the proprietor has given written notice, in the approved form, of the information specified in the Food Safety Standards that is to be notified to the appropriate enforcement agency before the business in conducted.

Expiation fee

If the offender is a corporate body: $1,500
If the offender is a natural person: $300

Furthermore, Section 86 (3) states if:
a) A food business is transferred to another person, or
b) There is a change in the name or address of a food business, the proprietor of the food
business (being, in the case where paragraph (a) applies, the new proprietor) must give
written notice, in the approved form and within 14 days, of the transfer or change (as the
case may be) to the appropriate enforcement agency.

Expiation fee: $250
